How to use this after a test comes back
  • Do it with the graded test in front of you, ideally within a day or two — you still remember what you were thinking on each question.
  • Be honest about the reason. "Careless" is the easy answer and it is often wrong. If you could not redo the question correctly right now with no help, it is a gap, not a slip.
  • One row per missed question, including partial credit. Add the topic if you know it — that is what builds the re-study list.
  • Print it and keep it. Bring the sheet to your next tutoring session or to your teacher's office hours; it turns "I did badly" into a specific list of what to work on.
